MYSTERY SHOPPER AGREEMENT: When it comes to mystery shopping, service savvy wants you to enjoy the assignments that you complete. We know that you are the most important piece in providing our clients with the objective feedback that they need to maintain and build a better business. We want you to be successful and have set forth the following guidelines and agreement to help you when completing assignments. MYSTERY SHOPPER RESPONSIBILITIES: A shopper must return a review within 24 hours after a scheduled shop. If for any reason you can't make a scheduled shop, contact us immediately so that we can reschedule. A shopper must follow the instructions of a shop. If you have any questions about an assignment, please contact us for clarification before you go on the shop. A shopper is to remain anonymous during a visit. Do not do anything to call attention to you. Handle your visit as though you are an everyday guest. Always get a copy of the itemized receipts. If the assignment covers the tip you can write it in next to the total on the receipt. Copies of receipts are due back within 24 hours of a shop and can be scanned and emailed or faxed to us. PLEASE NOTE THAT WE REQUIRE THE FOLLOWING WHEN SUBMITTING YOUR RECEIPT: The itemized receipt neatly copied at the upper right hand side of the submission. Use the space below to submit additional receipts. Make sure that the receipt is dark enough to copy or make adjustments accordingly. We must be able to read the copy of the itemized receipt that you submit. Your name and shopper id number at the upper left hand side of the submission. The name of the establishment followed by the location at the upper left hand side of the submission. The date of the shop at the upper left hand side of the submission. Please do not include any other information unless requested by a Service Savvy coordinator. RETURNING YOUR REVIEW: Our clients expect a prompt turn around of the review information that you're providing. You are an integral part of us meeting these deadlines. All reviews along with copies of receipts are due back to Service Savvy within 24 hours of an assignment. If a shopper fails to comply with our deadlines the shop may be rescheduled and you will not be reimbursed. REIMBURSEMENT: We reimburse shop expenses and pay shop fees within five weeks from the date that the shop was submitted. We offer the option of payment via Paypal for your convenience. The client sets forth reimbursement amounts. Some assignments cover the gratuity up to the reimbursement ceiling. We do not reimburse for portions of a tip over 20%. The reimbursement check will be mailed to the home address on file at Service Savvy. If you choose to be paid through the convenience of Paypal or to learn more about Paypal, go to www.paypal.com. It is a shopper's responsibility to inform Service Savvy in the event of an address or other personal information change so that we may update our records. We typically contact shoppers via email with shop information. Please look out for our monthly scheduling communication. Once you are scheduled be sure to look over all of the information as it relates to your assignment. You should look the information over in advance and let us know if you have any questions before going on the shop. When replying to schedule an assignment please make sure that you pick a specific date and day part (lunch/dinner/brunch) according to availability. Many locations have multiple shops in a given month and we need to coordinate precise dates and day parts. As many of the assignments that we schedule require a strict shopper rotation, we may not contact you every month. CANCELLATIONS, NO SHOWS AND LATE REVIEWS: If you need to reschedule a shop you must contact us immediately. The more time that you give us to reschedule the better. In the event that you are going to cancel an assignment you must contact us immediately. It is the discretion of Service Savvy to remove you from the database if cancellations become excessive. We do not allow more than 2 cancellations in a 6-month time frame. Any shopper that does not show up for a shop and does not inform us prior to the shop will be permanently removed from our list of mystery shopper candidates. No shows are simply unacceptable. If we have a problem receiving your review according to our deadline policy, we may not be able to use your review, reimburse you or schedule you for future assignments. TAX AND GENERAL LIABILITIES: You are responsible for reporting to the appropriate governmental agencies all amounts that you receive from us in the form of payment, and for paying all applicable federal, state and local taxes. If your payments from us exceed $599.99 in a calendar year, exclusive of reimbursable expenses, we will send a 1099 form. You agree not to hold Service Savvy liable for any damages you incur while on an assignment. You acknowledge and agree that you are not eligible for any benefits provided to our associates. This includes, but is not limited to, any worker's compensation or disability coverage provided to our associates. Except as otherwise expressly provided in the agreement, you will be responsible for all travel or business expenses you incur to complete an assignment, as well as for all travel or business expenses you incur for insurance coverage or licenses needed by you in order to perform an assignment. You guarantee that any vehicle that you operate during your assignment is fully insured to at least the minimum amounts required by law. You release Service Savvy from any and all claims arising out of your services and indemnify and hold Service Savvy not responsible from any use or misuse of products or services acquired by you in connection with your services. CONDUCT: You agree that you are responsible for your conduct, as well as the conduct of your children and guests while on an assignment. You agree to comply with all applicable laws, as well as the rules of any establishment while on assignment. We will have no liability to you or any third parties for any claims arising out of your performance on assignment. You agree that you indemnify and hold harmless Service Savvy for any such claims. While on an assignment, you will devote your full and undivided attention to the assignment until it is completed, and you will not do anything to impair the goodwill of Service Savvy or of our clients. You agree that you will not falsify any report, statement or your observations regarding any assignment. CONFLICT OF INTEREST: Because of our commitment to our clients to provide unbiased reports, you agree to immediately disclose any conflict of interest or potential conflict of interest that may preclude you from conducting an unbiased review for Service Savvy. You agree that after you have been accepted as a mystery shopper for Service Savvy, in the event that you discover you have a conflict of interest, you will volunteer such information to Service Savvy. CONFIDENTIALITY: As a mystery shopper for service savvy you acknowledge and agree that you will have information of confidential nature. You also agree that you will not disclose this information. This agreement also includes the terms that you will not contact any of our clients at any time for the purpose disclosing your relationship with us. You are not to duplicate any of service savvy's materials, or any part thereof, in any way or form. TERMINATION OF SERVICE: You understand that you are not guaranteed that you will be given an assignment. You also agree that we may terminate your service at any time, with or without cause. You certify that all information that you supply to Service Savvy is complete, accurate and true, and that falsification or failure to supply any requested information will result in the automatic termination of this agreement. You are free to provide your mystery shopping services to other companies however you agree that you will not function in any sales, administrative or functional service on a full or part time basis to any other firm that provides mystery shopping, advertising or market research business without the prior written consent of our president.
